Along with the child’s Health Booklet, it is a necessary document for the registration of students in the school and for the attendance in the Physical Education class.
The specializations of doctors who have the ability to issue and complete it and take the responsibility to sign it are pediatricians or general practitioners or internists in special cases (N.4238/2014). The Student Health Record Booklet is preventive and aims at protecting and safeguarding the health and life of male and female students and supporting them by taking preventive measures.
It includes a complete personal and family history, physical examination, optometry (visual acuity-strabismus control), somatometric control, vaccination coverage. Where necessary, there is a referral to laboratory, cardiology, and ophthalmology, orthopedic or other control. The duration of the Student Health Record Booklet is three years.
